Prior to your first appointment, please:

• First contact our office at 817.918.7300 to make sure we have availability that meets your needs.

• Click here to register as a new client in the Client Portal, and complete sections for your profile and contact information as well as any insurance you want to utilize.

• Complete all new client documents in the client portal. If you have any questions about any of the documents, please call our office to get clarification prior to signing the forms.

• Contact our our office to ask any other questions you may have and to schedule your first appointment.​
